Requirements:
- Active License in The State of Indiana
- DEA Certification
- Board Certification or Board Eligibility in Family Practice or Internal Medicine
Responsibilities:
- Flexible Schedule. Monday-Friday: Mon and Tue: 7:30a 6p, Wed and Th 7:30a-7p, Fri 7:45a-5p
- 4 or 5 Days a Week - At Least 36 Hours
- 20Patients a Day
- Call by Phone Every 2 Months
